 If for any reason you need a copy of your child's immunization record you can find it on the parent portal under the immunization tab. Once you have it located you can print it. 6th and 10th graders will need to have a Tdap immunization when they enter school in the fall. Does you child have theirs? |

B 2 MMR 1 Tdap If immunizations is against your religious beliefs, you must sign the religious exemption on the reverse side of the Colorado Department of Health Certificate of Immunizations. If your child cannot be immunized for medical reasons, a physician must sign the medical exemptions the back side of the certificate. If you have personal/philosophical belief opposed to immunizations, you must sign the personal exemption on the reverse side of the certificate. |
